Health & Wellness Review at home: $150 plus HST

This includes review of health history. Additional fees for other services.

Phone Health & Wellness Review: $75 plus HST

 

Prescription Refill: $25 plus HST

*This is offered following a Health & Wellness Review so that medical history, allergies etc. can be discussed prior to refill being given.

 

Bloodwork Requisition: $25 plus HST

*This is offered following a Health & Wellness Review so that medical history can be discussed prior to requisition being provided.

 

Pregnancy Support Package:

Prenatal and Pospartum support packages including home visits.

Examples of services provided include:

• An Introductory Consultation

• 2 Prenatal Home Visits

• 24/7 On-Call phone or email support 

• Immediate Postpartum Support

• 2 Postpartum Check-Ins

• Phone/Text Support for 6 Weeks Post Birth 

• Community Resource Recommendations

Postpartum Home Visit: 

Full physical assessment of mom and/or infant. Breastfeeding support.

Visits typically take 45-60 minutes.

These can be individual or group, in home or over a video call, or any other location.

3 HOME visits on your choice of days. $400 plus HST

Doctors Note: $20 plus HST

 

Fees are payable at the time of the visit by debit or credit card, email transfer or cash.

A receipt will be provided.
